FRAMES & LENSES

LENS ADD-ONS & UPGRADES

• Any lens enhancements or upgrades selected by either patient are not included

MEDICAL AID RULES

• Medical aid benefits may be claimed where applicable

• No codes may be altered to force eligibility

• Any shortfall or non-covered items are payable by the patient

SCRIPTS FROM OTHER OPTOMETRISTS

• External scripts must be clinically verified

• If an eye exam or re-check is required, it will be billed accordingly
