FRAMES & LENSES

• To qualify, the patient must purchase:

  • A qualifying frame

  • Clear CE lenses with prescription

• Photochromic lenses are provided at no extra cost as part of this promotion

• Any additional enhancements or add-ons beyond the photochromic upgrade are billed at standard pricing

• Premium lenses or other upgrades are not included in the promotion

ADD-ONS & UPGRADES

• Add-ons beyond the included photochromic upgrade (e.g., HMC, Blue-Safe, Transitions beyond qualifying range) are billed in full

• LIKE-FOR-LIKE rules do not apply to this promotion

MEDICAL AID RULES

• Medical aid benefits may be claimed where applicable

• Codes may not be altered to force eligibility

• Any shortfall or non-covered items are payable by the patient

SCRIPTS FROM OTHER OPTOMETRISTS

• All external scripts must be clinically verified

• If an eye exam or re-check is required, it will be billed accordingly
